1. Information We Collect

  • Information you give us directly: your name, email address, phone number, service interests, quiz responses, and any message you choose to send through our contact or subscription forms.

  • Information collected automatically: IP address, browser type, device type, referring page, and pages viewed. This is used for site performance and security, not for identifying you as a patient.

  • Clinical information: any protected health information (PHI) shared during a scheduled visit is collected and stored inside our HIPAA-compliant electronic health record and patient portal — not through this website.

2. HIPAA Compliance

  • NextDoor Telemedicine, PLLC is a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). We maintain administrative, physical, and technical safeguards designed to protect the confidentiality, integrity, and availability of protected health information.

  • Marketing forms on this website are intentionally limited to contact details and general interest. Please do not submit diagnoses, medications, or clinical history through website forms; use the secure patient portal instead.

  • We execute Business Associate Agreements (BAAs) with vendors who may handle PHI on our behalf. Vendors that handle only marketing contact data do not receive PHI.

  • We never sell protected health information, and we do not use PHI for marketing without your written authorization.

3. How We Use Your Information

  • To respond to inquiries, schedule care, and coordinate treatment.

  • To send marketing emails or SMS messages only where you have given explicit consent, which you may withdraw at any time by replying STOP or using the unsubscribe link.

  • To improve the website, our services, and our educational content.

  • To comply with legal, regulatory, and public-health obligations.

4. Data Security

  • Data submitted through this website is transmitted over encrypted TLS connections and stored in access-controlled databases with row-level security policies.

  • Access to identifiable information is limited to workforce members who need it to do their job, and access is logged.

  • No method of transmission over the internet is 100% secure. If we discover a breach affecting your unsecured PHI, we will notify you as required by the HIPAA Breach Notification Rule.

5. Your Patient Rights

  • Right to access and obtain a copy of your medical record.

  • Right to request an amendment to your record if you believe it is incorrect or incomplete.

  • Right to an accounting of certain disclosures of your health information.

  • Right to request restrictions on certain uses and disclosures.

  • Right to request confidential communications through an alternate address or phone number.

  • Right to receive a paper copy of our Notice of Privacy Practices.

  • Right to file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights without fear of retaliation.

6. Cookie Policy

  • We use strictly necessary cookies to keep the site functioning, and may use analytics cookies to understand aggregate traffic patterns.

  • We do not place advertising cookies that combine your browsing behavior with protected health information.

  • You can block or delete cookies through your browser settings; core site functions will continue to work.

7. Third Parties and Marketing Platforms

  • Email and SMS marketing may be delivered through a third-party platform. Only the contact details and consent flags you provide are shared with that platform.

  • Scheduling and clinical documentation occur in our HIPAA-compliant portal, which is governed by its own security controls and our BAA with that vendor.

8. Children's Privacy

  • This website is not directed to children under 13, and we do not knowingly collect information from them through the site.

9. Changes to This Policy

  • We may update this policy to reflect changes in our practices or the law. The effective date below will always show the current version.
