1. Information We Collect

Information you give us directly: your name, email address, phone number, service interests, quiz responses, and any message you choose to send through our contact or subscription forms.
Information collected automatically: IP address, browser type, device type, referring page, and pages viewed. This is used for site performance and security, not for identifying you as a patient.
Clinical information: any protected health information (PHI) shared during a scheduled visit is collected and stored inside our HIPAA-compliant electronic health record and patient portal — not through this website.

NextDoor Telemedicine, PLLC is a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). We maintain administrative, physical, and technical safeguards designed to protect the confidentiality, integrity, and availability of protected health information.
Marketing forms on this website are intentionally limited to contact details and general interest. Please do not submit diagnoses, medications, or clinical history through website forms; use the secure patient portal instead.
We execute Business Associate Agreements (BAAs) with vendors who may handle PHI on our behalf. Vendors that handle only marketing contact data do not receive PHI.
We never sell protected health information, and we do not use PHI for marketing without your written authorization.

To send marketing emails or SMS messages only where you have given explicit consent, which you may withdraw at any time by replying STOP or using the unsubscribe link.
